If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out before your first appointment. Printing the forms, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. FINANCIAL INFORMATION

Our dental office is primarily a fee-for-service practice. However, patients with any PPO Dental Reimbursement Policy will have a benefit at our office and we will help you get reimbursement for covered procedures.

Our practice does contract with Delta Dental Premier, Cigna Radius and Anthem Blue Cross 200 and 300.
We accept Master Card, Visa and Discover as a convenience for our patients.

We offer patients Care Credit which gives you flexibility when managing your out-of-pocket health care expenses. You can access Care Credit’s website application process at the link below.
